Replacement ID Card for Student/Parent/Guardian

Loss of ID card(s) for Student/ Parent/Guardian

There is a $50 replacement fee (inclusive of 7% GST) for the loss of the student or Parent/Guardian Security ID Card.

The process for replacement is as follows:

1. Duly fill the online form with all the details, print the form and submit it to the Finance Office.

2. You will need to pay $50 (by cash or cheque) when the form is submitted to Finance and you will be issued with an official receipt after payment is made.

3. Proceed to the IT Office with the official receipt for verification. Processing time will be between 3 to 5 working days.

4. The ID card(s) will be hand over to respective School Secretaries who will hand the card(s) to your child’s Homeroom Teacher who, in turn, will pass them over to your child.
